We know that your health problems may present you with challenges of getting to the doctor's office for care. With House Call Medical Services, we provide primary care health services in the home for your convenience.

- Order, perform and interpret diagnostic tests such as lab work and x-rays, flu shots, pneumonia vaccines, and all other injections.
- Diagnose and treat acute and chronic conditions such as diabetes, high blood pressure, infections, and injuries.
- Prescribe medications and other treatments.
- Perform examinations and make diagnosis.
- Order and interpret test.
- Perform procedures.
- Write prescriptions and medication assessments.
- Provide referrals to community resources and specialists.
- Assess environmental and living environment.
- Customize a plan of care that addresses medical, social/environmental, and emotional concerns.

Services for Clinics/ Doctors’ Offices
- Manage a patient’s care while maintaining continuous communication with the primary care physician.
- Our Nurse Practitioners can provide telephonic management of acute and chronic signs and symptoms.
- They can discern a diagnosis quickly and accurately using evidence-based practice models in delivering safe, quality, effective care.
- They can prescribe appropriate medications and advise follow-up.
- They can evaluate and refer to the facility of your choice if emergent care is warranted.
NOTE: These services may be performed in the home setting, whether it is an assisted living facility, skilled nursing facility or in the comfort of the individual’s home. Once a home visit has been made, visit note and any other medical records will be sent to the primary care physician of record.
